Medical Corporation Reinstatement Application

To apply for a Certificate of Authorization for a medical corporation that was previously authorized by the Board, please download and complete the medical corporation reinstatement application.

In association with the reinstatement application, the corporation will need to pay the $500 application fee. The Board will send payment instructions via email following receipt of the application.
