Controlled Substance Dispensing Practitioner Registration Renewal For Medical Doctors Whose Last Names Begin With The Letters A Through L

Effective at 11:59 pm Eastern Daylight Time (EDT) on June 30, 2026, all controlled substance dispensing practitioner registrations for medical doctors whose last names begin with the letters A through L will expire unless successfully renewed prior to 4:30 pm EDT on June 30, 2026. The Board strongly encourages early renewal to avoid unexpected problems or delays in renewal completion.   

As a reminder, a controlled substance dispensing practitioner registration is only required if the physician administers and/or dispenses controlled substances in an office-based practice. It is not required for (1) inpatient hospital practice; or (2) to write prescriptions which are to be dispensed by a pharmacy. Physicians who will not be administering and/or dispensing controlled substances in an office-based setting after June 30, 2026 do not need to take any action and his/her registration will simply expire.

Renewal Process

To renew a controlled substance dispensing practitioner registration for the period of July 1, 2026 to June 30, 2028 physicians must:

  1. Successfully renew his/her medical license.  
  2. Download and complete the 2026 Controlled Substance Dispensing Practitioner Registration Renewal Application.
  3. Submit the completed renewal application to the Board via US Mail or email.

Renewal Fee

The renewal application fee is $30 per dispensing location. Do not include the renewal application fee with the application. Upon receipt of a completed renewal application, the Board will send payment instructions to the email address of record with the Board. Applications are incomplete until payment of the renewal application fee is received.
