All credentials issued by the West Virginia Board of Medicine are available on the Board’s website. Licenses, including temporary licenses, are included on a practitioner’s website profile, which can be accessed through the provider search feature. Spreadsheets are available for Educational Permits, Interstate Telehealth Registrations and Emergency Registrations issued during the COVID-19 State of Emergency. Make payment and provide the name of the recipient state board. Board staff will contact you via email at the address you provide to obtain delivery instructions for the selected state board. Please complete the form and payment for each individual verification.
