Foreign Medical Corporation Application Instructions

 

A medical corporation authorized and in good standing in another state is eligible to apply for a Certificate of Authorization issued by this Board to practice medicine or podiatric medicine to patients located in West Virginia if the foreign medical corporation shareholders are all licensed physicians (allopathic, osteopathic or podiatric) or physician assistants, and at least one shareholder holds an active status license issued by this Board that is in good standing. 

Prior to completing the Board’s foreign medical corporation application, please check with the West Virginia Secretary of State to confirm that the intended name of your corporation complies with state regulations.

To apply for a Certificate of Authorization for a foreign medical corporation, please download and complete the foreign medical corporation application. In association with the application, the corporation will need to:

  • Submit a copy of the proposed Articles of Incorporation that the corporation will file with the West Virginia Secretary of State.
  • Submit documentation which demonstrates that the medical corporation is authorized and is currently in good standing in its state of incorporation.
  • Pay the $500 application fee. The Board will send payment instructions via email following receipt of the application.
