Controlled Substance Dispensing Practitioner Registration Renewal For Medical Doctors Whose Last Names Begin With The Letters A Through L
Effective at 11:59 pm Eastern Daylight Time (EDT) on June 30, 2024, all controlled substance dispensing practitioner registrations issued to medical doctors whose last names begin with the letters A through L will expire unless successfully renewed prior to 4:30 pm EDT on Friday, June 28, 2024. The Board strongly encourages early renewal to avoid unexpected problems or delays in renewal completion.
As a reminder, a controlled substance dispensing practitioner registration is only required if the physician administers and/or dispenses controlled substances in an office-based practice. It is not required for (1) inpatient hospital practice; or (2) to write prescriptions which are to be dispensed by a pharmacy. Physicians who will not be administering and/or dispensing controlled substances in an office-based setting after June 30, 2024 do not need to take any action and his/her registration will simply expire.
Renewal Process
To renew a controlled substance dispensing practitioner registration for the period of July 1, 2024 to June 30, 2026 physicians must:
- Successfully renew his/her medical license.
- Download and complete the 2024 Controlled Substance Dispensing Practitioner Registration Renewal Application.
- Submit the completed renewal application to the Board via US Mail or email.

Renewal Fee
The renewal fee is $30 per dispensing location. Upon receipt of a completed renewal application, the Board will send payment instructions to the email address of record with the Board. Applications are incomplete until payment of the renewal application fee is received.
