 Medical Doctors Whose Last Names Begin with the Letters A Through L

Effective at 11:59 pm Eastern Daylight Time (EDT) on June 30, 2024, all current controlled substance dispensing practitioner registrations for physicians whose last names begin with the letters A through L (“A through L physicians”) will expire unless successfully renewed during the Board’s renewal period. Controlled substance dispensing practitioner registration renewal is separate and distinct from medical license renewal.

Beginning on May 1, 2024, the 2024 Controlled Substance Dispensing Practitioner Registration Renewal Application will be available for download on the Board’s website.   Registration renewal ends at 4:30 pm EDT on Friday, June 28, 2024. To avoid registration expiration, the Board must receive the complete application and payment of the renewal fee by the renewal deadline.  Because there is no grace period for office-based dispensing and/or administration of controlled substances after expiration, the Board strongly encourages early renewal to avoid unexpected problems or delays in renewal completion.

  1. What is a controlled substance dispensing practitioner registration?   

Practitioners who dispense and/or administer controlled substances, including free samples of controlled substances, in an office-based setting must be registered with the Board as a controlled substance dispensing practitioner. To apply for an initial registration or for other questions regarding controlled substance dispensing practitioner registration, please email the Board at wvbom@wv.gov.

  1. Am I currently registered as a controlled substance dispensing practitioner?

If you do not know if you are currently registered as a  controlled substance dispensing practitioner, use the Board’s “Look up a Doctor or PA” search feature to review your license profile. If you have an active controlled substance dispensing registration, it will be listed under your license number.  If you call the Board to determine your registration status, you will be redirected by Board staff to the website search feature.

  1. Do I need to renew my controlled substance dispensing practitioner registration with the Board in 2024?

If you are a medical doctor whose last name begins with the letter A through L and you intend to continue to dispense and/or administer controlled substances, including free samples of controlled substances, in an office-based setting after June 30, 2024, you must be registered with the Board as a controlled substance dispensing practitioner and you need to apply for renewal for each of your West Virginia office-based dispensing locations. 

  1. How do I renew my controlled substance dispensing practitioner registration with the Board?

To renew a controlled substance dispensing practitioner registration for the period of July 1, 2024 through June 30, 2026:

1.         Renew your medical license. You must successfully renew your medical license before your Controlled Substance Dispensing Practitioner Registration Renewal Application will be processed.

2.         Download and complete the 2024 Controlled Substance Dispensing Practitioner Registration Renewal Application.

3.         Submit the completed renewal application to the Board via US Mail or email.

  1. Can I renew online?

No.  The controlled substance dispensing practitioner registration renewal process for this cycle requires the submission of a paper application and payment of the associated fee. The renewal application will be available for download on the Board’s website beginning May 1, 2024.

  1. What is the renewal deadline for 2024?

Completed renewal applications and payment of the appropriate fee must be received by the Board before 4:30 pm EDT on Friday, June 28, 2023.

  1. Who can renew my controlled substance dispensing practitioner registration for me?

You are the only person authorized to renew your dispensing registration.  The Board’s renewal application includes several verifications and an attestation that require you to personally complete your renewal application. Allowing anyone else to complete the verifications and/or attestation on your behalf can constitute misconduct and/or fraud. 

  1. How much does it cost to renew my controlled substance dispensing practitioner registration?

The biennial renewal fee is $30 for each dispensing location. Once renewed, your registration will be valid for each dispensing location from July 1, 2024 through June 30, 2026.

  1. Will I need to provide any documents with my renewal application?

No.  All you need to submit is your completed two-page application and payment of the appropriate fee.

  1. What information do I need to have available when I fill out my renewal application?

You will need to know:

  1. Your WVBOM license number;
  2. Your DEA registration number;
  3. The physical addresses for all of your office-based controlled substance dispensing/administering locations; and

You will also need to verify that:

  1. You are currently registered to access the West Virginia Controlled Substance Monitoring Program (WVCSMP) Database;
  2. The DEA controlled substance registration number you provided, is valid, unexpired and is not subject to any restrictions or limitations;
  3. You have never pled guilty (or no contest) to, and have never been adjudged guilty of, a felony relating to controlled substances in any jurisdiction;
  4. You are not subject to any administrative or court order in any jurisdiction which places restrictions or limitations of any kind upon your prescriptive authority and/or ability to prescribe; and
  5. You understand and agree to comply with your obligation to report the dispensing of controlled substances to the WVCSMP.
  1. How can I find my WVBOM license number?

If you do not remember your WVBOM license number, use the “Look up a Doctor or PA” search feature located on the Board’s website to obtain your number. If you call the Board for your license number, you will be redirected by Board staff to the website search feature.

  1. Where can I find my DEA number?

Your DEA registration number is on the top left-hand side of your Controlled Substance Registration Certificate issued by the US Department of Justice Drug Enforcement Administration.  DEA certificate holders may also look up registration information at www.dea.gov.  Do not contact the Board for your DEA number.

  1. I don’t have a current DEA registration.  Can I still renew my controlled substance dispensing practitioner registration?

No. You must have a valid unrestricted DEA number to renew your registration as a controlled substance dispensing practitioner.

  1. How do I get my renewed controlled substance dispensing practitioner registration certificate(s)?

Once your renewal application has been processed, your controlled substance dispensing practitioner registration expiration date will be updated to June 30, 2026 on your Board of Medicine website profile.  The renewal certificate(s) will be sent to your mailing address on record with the Board.  If you have registered multiple locations, you are responsible for ensuring that each certificate is maintained at the correct dispensing location.

  1. I only dispense samples of controlled substances.  Do I need to renew my registration as a dispensing practitioner?

Yes.  You must register as a controlled substance dispensing practitioner if you dispense and/or administer any controlled substances, including free samples of controlled substances, in an office-based setting.

  1. I only dispense and/or administer non-controlled substances from my office.  Do I need to renew my dispensing practitioner registration?

No. You are not required to register with the Board if you only dispense and/or administer non-controlled substances.  If you no longer need to be registered with the Board, simply let your current registration expire.

  1. I only dispense and/or administer controlled substances to hospital patients.  Do I need to register as a controlled substance dispensing practitioner?

No.  You are not required to register with the Board unless you dispense and/or administer controlled substances in an office-based setting.

  1. My controlled substance dispensing practitioner registration renewal application was returned to me as incomplete, and I did not have time to get the completed application to the Board before the renewal deadline. Can I still dispense and/or administer controlled substances while my application to restore my dispensing registration is being processed?

No.  Your dispensing registration expired, effective 11:59 pm on June 30, 2024, and you are not authorized to dispense and/or administer controlled substances in an office-based setting at this time. There is no grace period for dispensing controlled substances after your registration expires.

  1. My controlled substance dispensing practitioner registration expired because I did not submit my renewal application to the Board before 4:30 pm on Friday, June 28, 2024.  What does this mean for my practice?

Your dispensing registration expired, effective 11:59 pm on June 30, 2024, and you are not authorized to dispense and/or administer controlled substances in an office-based setting at this time. There is no grace period for dispensing controlled substances after your registration expires.

  1. My controlled substance dispensing practitioner registration has expired.  How do I get a new one?

You must apply anew by submitting an initial controlled substance dispensing practitioner registration application to re-register. The initial application is available on the Boards website.
