Domestic Medical Corporation Application Instructions

To qualify for a Certificate of Authorization for a domestic medical corporation, all shareholders must hold an active status West Virginia license and be in good standing with the Board.

The proposed name of your medical corporation shall contain one of the following endings:

Corporation
Corp.
Incorporated
Inc.
Company
Co.
Limited
Ltd.

 West Virginia does not permit the authorization of “Professional Corporations” or “P.C.”.

To apply for a Certificate of Authorization for a domestic medical corporation, please complete the online medical corporation application. During the application process, the corporation will be required to:

• Print the corporation shareholder page and have each shareholder sign the page. The  signed shareholder page must be uploaded in association with the application process.  Please note that the Board does not accept stamped or electronically generated signatures.
• Upload a copy of the proposed Articles of Incorporation that the corporation will file with the West Virginia Secretary of State.
• Pay the $500 application fee via credit card at the conclusion of the application process.
