Professional Limited Liability Company Reinstatement Application

 

To apply for a Certificate of Authorization for a professional limited liability company that was previously authorized by the Board, please download and complete the professional limited liability company reinstatement application.

All members of the professional limited liability company must be a licensed physician (allopathic, osteopathic or podiatric), and all members must hold an active status West Virginia license that is in good standing.

In association with the reinstatement application, the company will need to:

  • Submit a copy of the most recent Annual Report that the company filed with the West Virginia Secretary of State.
  • Submit proof of current professional liability insurance in the amount of one million dollars in the business name of the company.
  • Pay the $100 application fee. The Board will send payment instructions via email following receipt of the application.
