All medical school graduates participating in graduate medical training in West Virginia must have a West Virginia medical license or an educational permit issued by the Board. This applies to medical school graduates participating in residency (including intern year) and fellowship training. It also applies to graduate medical training participants in other states who intend to complete training rotations within West Virginia.

No. The Board needs your original application, with your personal signature and the personal signature of your program director/authorized designee. Additionally, your application contains personally identifiable information, such as your date of birth and Social Security number, and should never be emailed.
You can apply any time in advance of your training commencement date. The Board recommends that you apply at least three weeks in advance of your training start date. For physicians who will be participating in graduate medical training on July 1, 2024, please apply early. June is an extremely busy month for the Board, and last minute applicants should expect a delay in the processing of permit applications.
All educational permits with an issue date of July 1, 2024 or after will expire on June 30, 2025, unless the permit holder leaves or transfers from his or her program, completes training, or is granted medical licensure prior to that date. A permit is not transferrable from program to program.
Yes. The non-refundable application fee is $100. Do not submit payment with your application. Upon receipt of your completed application, the Board will send payment instructions to the email address provided on the application. Your application will not be processed until payment is received.

The permit applicant must complete Section A. The program director may delegate completion of Section B to a designee by submitting an original signed letter to the Board which identifies the full name and title of the designated individual. Only one person may be the authorized designee for the program, and that individual will be the point of contact for the Board with respect to all educational permit applications for the program.
No. The educational permit application has two sections. You must complete Section A yourself. Section B must be completed by your program director or his or her Board authorized designee after you complete Section A. The application, including personal signatures in both sections, must be submitted to the Board.
No. Among other things, the program director/authorized designee is certifying the information provided by the applicant in Section A. Applications which are completed in the wrong order will be rejected and must be resubmitted after completion in the appropriate order.
Your graduation date appears on your medical school diploma. If you are providing an anticipated date of graduation, please verify the anticipated date with your medical school. The Board may require you to submit a copy of your medical school diploma if your medical school graduation date appears incorrect or incomplete.
Disclosing your Social Security number is mandatory for the Board to comply with the reporting requirements of the federal National Practitioner Data Bank. However, to facilitate the permitting process, you may submit your application pending receipt of a Social Security number. You must provide your Social Security number to the Board as soon as you receive a number, or no later than 60 days after your training has begun. If you do not have a Social Security number within 60 days, your permit will expire until a Social Security number is provided. Due to privacy concerns, the Board will only accept your Social Security number via mail, preferably certified mail. Please do not email or call the Board with your Social Security number.
The Board will inform you and your program director/authorized designee via email. A copy of your permit will also be provided via email to you and your program director/authorized designee.

Your training program establishes the scope of your training. However, if your program permits prescribing, you must successfully complete a minimum of three hours in a Board-approved course on drug diversion training and best practice prescribing of controlled substances training CME before you prescribe any controlled substances. A list of approved courses is available on the Board’s website.
Yes. You must notify the Board within five days of leaving your training program for any reason other than successful completion of the contract term. Your educational permit becomes invalid when you leave your training program.
