Effective at 11:59 pm Eastern Daylight Time (EDT) on March 31, 2025, all current controlled substance dispensing practitioner registrations for physician assistants will expire unless successfully renewed during the Board’s renewal period. This is separate and distinct from your physician assistant license renewal.
On January 31, 2025, the Board will mail a 2025 Controlled Substance Dispensing Practitioner Registration Renewal Application to all physician assistants who are currently registered to dispense controlled substances in an office-based setting. To avoid registration expiration, the Board must receive the completed application and payment of the renewal fee by 4:30 pm EDT on March 31, 2025. Because there is no grace period for office-based dispensing and/or administration of controlled substances after expiration, the Board strongly encourages early renewal.

Renewal FAQs for Physician Assistants
Physician assistants who administer and/or dispense controlled substances (including free samples) in an office-based setting must be registered with the Board as a Controlled Substance Dispensing Practitioner. Please note that this credential is only required if you administer and/or dispense controlled substances in an office-based practice. It is not required for: (1) inpatient hospital practice; or (2) to write prescriptions which are dispensed by a pharmacy. To renew as a Controlled Substance Dispensing Practitioner, first renew your physician assistant license, then submit a 2025 Controlled Substance Dispensing Practitioner Registration Renewal Application and pay the associated renewal application fee before 4:30 pm on March 31, 2025. If you do not know if you are currently registered as a Controlled Substance Dispensing Practitioner, use the Board’s “Look up a Doctor or PA” search feature to review your license profile. If you have an active Controlled Substance Dispensing Registration, it will be listed under your license number. If you call the Board to determine your registration status, you will be redirected by Board staff to the website search feature.
If you are a physician assistant and you intend to dispense and/or administer controlled substances, including free samples of controlled substances, in an office-based setting after March 31, 2025, you must be registered with the Board as a controlled substance dispensing practitioner and you need to apply for renewal for each of your West Virginia office-based dispensing locations.
To renew a Controlled Substance Dispensing Practitioner Registration for the period of April 1, 2025 through March 31, 2027:
1. Renew your physician assistant license. You must successfully renew your license before your dispensing practitioner registration renewal application will be processed.
2. Complete the Controlled Substance Dispensing Practitioner Registration Renewal Application that was mailed to you on January 31, 2025.
3. Submit the original completed renewal application to the West Virginia Board of Medicine, via US mail, early enough that it arrives before the renewal deadline of 4:30 pm on March 31, 2025.
The renewal application fee is $30 per dispensing location. Do not include the renewal application fee with the application. The West Virginia Board of Medicine accepts online credit card payments for all fees. Upon receipt of the renewal application, the Board will send payment instructions via email. Your application is incomplete until payment of the renewal application fee is received.

You are the only person authorized to renew your Controlled Substance Dispensing Practitioner Registration. The Board’s renewal application includes several verifications and an attestation that require you to personally complete and submit your renewal application. Allowing anyone else to complete the verifications and/or attestation on your behalf can constitute misconduct and/or fraud.
The biennial renewal fee is $30 for each dispensing location. Once renewed, your registration will be valid for each dispensing location from April 1, 2025 through March 31, 2027.

You will need to know:
a. Your WVBOM physician assistant license number;
b. Your DEA registration number; and
c. The physical addresses for all of your office-based controlled substance dispensing/administering locations.
You will also need to verify that:
1. You are currently registered to access the West Virginia Controlled Substance Monitoring Program (WVCSMP) Database;
2. The DEA controlled substance registration number you provided, is valid, unexpired and is not subject to any restrictions or limitations;
3. You have never pled guilty (or no contest) to, and have never been adjudged guilty of, a felony relating to controlled substances in any jurisdiction;
4. You are not subject to any administrative or court order in any jurisdiction which places restrictions or limitations of any kind upon your prescriptive authority and/or ability to prescribe; and
5. You understand and agree to comply with your obligation to report the dispensing of controlled substances to the WVCSMP.

Your DEA registration number is on the top left-hand side of your Controlled Substance Registration Certificate issued by the US Department of Justice Drug Enforcement Administration.DEA certificate holders may also look up registration information at www.dea.gov. Do not contact the Board for your DEA number.
Once your renewal application has been processed, your controlled substance dispensing practitioner registration expiration date will be updated to March 31, 2027 on your Board website profile. Renewal certificate(s) will be sent to your mailing address on record with the Board. If you have registered multiple locations, you are responsible for ensuring that each certificate is maintained at the correct dispensing location.
