Initial Application - Medical Doctor

Welcome to the WV Board of Medicine Licensure Portal

You have completed the Uniform Application portion of the process and will now complete your application for either an initial medical license or the reactivation of a previously held license that has been expired for more than one year.

  1. On the following screen, you will find a partial summary of information you provided in your Uniform Application. Upon review, if you discover errors, please return to the Uniform Application for correction before proceeding to the next stage of your application. You will be asked to answer fifteen (15) professional practice questions and three (3) required child support information questions.

    Please note the time period for the Questions 1 - 12 asks "Have you ever..." and for questions 13 - 15 asks "Have you in the last five years..." A "yes" response to any question will require a typed explanation in the text box provided, and may also require additional information to be uploaded and submitted as described with each question.


    If information is incomplete or inaccurate, this will delay the processing of your application.
  2. If you are unable to complete this portion of the application in one sitting, you may use the link and Application ID that was sent to you via e-mail by the Federation of State Medical Boards (FSMB) to return to this screen.
  3. After completing the last screen for certification of your application, you will be directed to the WV Treasurer's website to pay the $400 application fee by credit card. You will receive a receipt from the WV Treasurer via the e-mail you provide on your payment information page. Please open the receipt attachment titled "payment confirmation" and check that the payment status is "completed OK" and not "failed to complete." If your payment has failed we will not receive your application. You may enter this portal again as described in the previous paragraph. All application fees are non-refundable. If you have doubts about meeting licensure requirements, please contact the Board before finalizing payment.
  4. After payment has been received, your application will be pending review. You will receive an e-mail notification when it has been initially reviewed. Once we receive your Affidavit along with Addendum 1 in our office; you will be sent an e-mail detailing the status of your application within three (3) weeks of receipt of these two documents. Common errors on these forms that delay processing are: submitting an old photo, not dating or signing the notarized Affidavit form, and omitting the photo declaration date on Addendum 1.
