2018 PLLC Renewal Instructions
Your certificate of authorization as a PLLC is scheduled to expire on June 30, 2018. The PLLC renewal process for this cycle requires the submission of a paper application and associated documentation.
The Board mailed an application for the renewal of your PLLC to your mailing address of record on April 23, 2018. To avoid the expiration of your certificate of authorization, the Board must receive your completed application, with all associated documents and the required renewal fee, before 4:30 pm on Friday, June 29, 2018.
Failure to submit a complete renewal application by the deadline will result in the expiration of the PLLC’s certificate of authorization issued by the Board effective 11:59 p.m. EDST on Saturday, June 30, 2018, which will require the PLLC to cease rendering professional services in the State of West Virginia.
Please note that you must provide a current e-mail address for the PLLC on your renewal application. All current PLLC members must sign the application, and the certifying signature must be provided by a member of the PLLC, not the office manager.
Along with your completed paper application, you must submit:
- A copy of the PLLC’s Annual Report for the previous year which the PLLC filed with the Secretary of State;
- A copy of the PLLC’s certificate of professional liability insurance, identifying the PLLC as the insured entity, which demonstrates continued insurance coverage for the PLLC in the amount of $1,000,000.00;
- All membership changes in the PLLC between July 1, 2017 and the present. If any physicians or podiatric physicians have relinquished or otherwise ended their membership in the PLLC between July 1, 2017 and the present, you must provide (on a separate sheet of paper) a list of all individuals who have left the membership of the PLLC in the last year, and their specific date(s) of departure; and
- The PLLC renewal fee of $100.00 made payable to the West Virginia Board of Medicine.
Accuracy and completeness are important to ensure the efficient processing of your renewal application. All incomplete applications will be returned to the renewal applicant’s mailing address unprocessed. Missing contact information, signatures, etc. will cause the return of your application and delay the process. Thank you for your cooperation.
