2022 PLLC Renewal

Effective at 11:59 pm Eastern Daylight Time (EDT) on June 30, 2022, all current certificates of authorization will expire unless successfully renewed prior to 4:30 pm EDT on Thursday, June 30, 2022. The Board strongly encourages early renewal to avoid unexpected problems or delays in renewal completion.   

On May 2, 2022, the Board mailed a PLLC renewal application to each PLLC. Along with the completed application, the PLLC must submit:

  1. A copy of the most recent Annual Report that the PLLC filed with the West Virginia Secretary of State;
  2. A copy of the PLLC’s certificate of professional liability insurance, identifying the PLLC as the insured entity, which demonstrates continuous insurance coverage for the PLLC in the amount of $1,000,000.00; and
  3. If applicable, a written statement which identifies any former member who left the PLLC after July 1, 2021 by full name and the specific date (month, day and year) of departure from the PLLC;

The renewal application fee is $100.  Please do not include the renewal application fee with the application.  The West Virginia Board of Medicine accepts online credit card payments for all fees.  Upon receipt of the renewal application, the Board will send payment instructions via email.  Your application is incomplete until payment of the renewal application fee is received.

To avoid expiration, the Board must receive the completed PLLC renewal application, with all associated documents prior to 4:30 pm on June 30, 2022.  If the Certificate of Authorization expires, the PLLC must cease rendering professional services through the company in the State of West Virginia.
