2022 Controlled Substance Dispensing Practitioner Registration Renewal for Physicians Whose Last Names Begin with the Letters A through L 
 
Controlled Substance Dispensing Practitioner Registrations for physicians whose last names begin with the letters A through L are scheduled to expire at 11:59 pm Eastern Daylight Time (EDT) on June 30, 2022. There is no grace period for office-based dispensing and/or administering after the registration expires.  All practitioners who engage in office-based dispensing and/or administering of controlled substances, including free samples of controlled substances, must be registered with the Board as a Controlled Substance Dispensing Practitioner.  
 
To renew a Controlled Substance Dispensing Practitioner Registration for the period of July 1, 2022 through June 30, 2024:
 
1. Renew your medical license. You must successfully renew your license before your dispensing practitioner registration renewal application will be processed. 
 
2. Download and complete the Controlled Substance Dispensing Practitioner Registration Renewal Application. 
 
3. Submit the original completed renewal application via US mail, early enough that it arrives before the renewal deadline of 4:30 pm on Thursday, June 30, 2022
 
The renewal application fee is $30 per dispensing location.  Please do not include the renewal application fee with the application.  The West Virginia Board of Medicine accepts online credit card payments for all fees.  Upon receipt of the renewal application, the Board will send payment instructions via email.  Your application is incomplete until payment of the renewal application fee is received.
