2019 PLLC Renewal


Effective at 11:59 pm Eastern Daylight Savings Time (EDST) on Sunday, June 30, 2019, all current Certificates of Authorization for Professional Limited Liability Companies will expire unless successfully renewed prior to 4:30 pm EDST on Friday, June 28, 2019.  

On April 29, 2019, the Board will mail a renewal application to each PLLC. Along with the completed renewal application, the PLLC must submit:

  1. A copy of the 2018 Annual Report that the PLLC filed with the West Virginia Secretary of State;
  2. A copy of the PLLC’s certificate of professional liability insurance, identifying the PLLC as the insured entity, which demonstrates continued insurance coverage for the PLLC in the amount of $1,000,000.00;
  3. If applicable, a written statement which identifies any former member who left the PLLC after July 1, 2018 by full name and the specific date (month, day and year) of departure from the PLLC; and
  4. The PLLC renewal fee of $100.00 payable to the West Virginia Board of Medicine.

Accuracy and completeness are important to ensure the efficient processing of the renewal application. Missing contact information, signatures, etc. will cause the renewal application to be returned to the PLLC unprocessed.

To avoid expiration the Board must receive the completed PLLC renewal application, with all associated documents and the renewal fee, prior to 4:30 pm EDST on Friday, June 28, 2019. If the Certificate of Authorization expires, the PLLC must cease rendering professional services in the State of West Virginia.
